This page covers what you need to prepare for your first appointment, including required patient forms, insurance and payment information, and how to contact our team with questions. 

We are in network with:

• Aetna (including Aetna CVS; except Duke Basic/Select)
• NC State Health Plan
• Blue Cross Blue Shield "BCBS" (except Blue Local)
• UnitedHealthcare (UHC), UMR, Surest, and other UHC affiliates

For insurance plans with whom we are in network, we will submit claims on your behalf after your appointment.  Please see the link to the right to find out if we are in network with your insurance plan and if your plan covers nutrition services, and if so, the details of your coverage.

For self‑pay clients, we can provide a superbill to submit to your insurance company for possible reimbursement.  Payment is due at the time of service.  If you submit a superbill, reimbursement will be sent directly to you from your insurance company.
